Client Rights and Grievance Form

To ensure that a client of A Village for One will be an active, informed participant in their plan of care, the client and guardian are empowered with certain rights and responsibilities.

Any client and/or guardian can file a complaint following the grievance procedures outlined in the policy by using our Client Grievance Form.
